Question:
To ask the Minister of Health to detail any safeguards in place to ensure that patients transferred between Health and Social Care Trusts do not experience delays in follow-up care or aftercare in their home Health and Social Care Trust.

Answer:

The onus is on the discharging Health and Social Care Trust to make a timely referral as appropriate to the home Health and Social Care Trust.

The ability to provide care in the home Trust will be dependent on service capacity within the home Trust at the time of the referral.

The individual is central in this process and it is important that this is done in a timely way to ensure the least possible inconvenience to the person and or their family
