To ask the Minister of Education, pursuant to AQW 23755/22-27, to detail the actions relating to transitions planning for young people with a statement of special educational need due to leave statutory education that are contained in the Special Educational Need Reform Agenda Delivery Plan 2025-30.

Answer:

Under the pillar ‘Right Time’ in the SEN Reform Agenda and Delivery Plan there is the overarching action for a ‘Transitional Support Programme at various stages of learner’s journey’, with a sub-action to ‘Work with Health, Economy and Communities to develop appropriate pathways into adulthood for school leavers with SEN’.
